Arizona state law requires that all children must have proof of immunizations, proof of immunity or a valid exemption on file in order to attend school. The immunization record of each vaccine dose must include the full date the vaccine was administered and the name of the doctor or clinic.

Immunization Clinic Locations

Below are some immunization clinic locations. Please call ahead to verify clinic dates, locations, eligibility and if they have the vaccine you need. You must bring all immunization records with you to the clinics and any immunization referral paperwork indicating which immunizations are needed. Please note that most clinics are by appointment only due to Covid-19.
