The Arizona Department of Health Services requires that children at age 11 receive updated immunizations in order to remain compliant and attend school in-person in Arizona. The required immunizations needed on the 11th birthday (or within 14 days after their 11th birthday) are as follows:
- Students 11 years of age and older will be REQUIRED to have one dose of Tdap vaccine if 5 years have passed since their last dose of Tetanus containing vaccine.
- Students 11 years of age and older will be REQUIRED to have one dose of Quadrivalent Meningococcal (MenACWY or MCV4) vaccine.
Students will have 14 days to get the required immunization after they turn 11 years old to remain in compliance with the new rules. Students without the required vaccines will be excluded from attending school until they receive the required vaccines and submit the required paperwork to the health office. Please bring documentation into the health office as soon as the immunizations are received or fax records to (480)224-9416.
If you have any questions regarding these rules, please feel free to call the health office at (480)883-5902.
