Medical and Dental

The Chandler CARE Center is a 501c3 nonprofit dedicated to providing access to no-cost medical and dental care to uninsured Chandler youth.

 

Access to medical and dental care for children is possible thanks to our partnership with Dignity Health.  The CARE Center is a Qualified Charitable Organization, and your financial contribution to support helps make this no-cost program for children in our community possible, and may qualify as a tax credit in the state of Arizona.

Program Information

To be eligible for care, you must:

• Live in the city of Chandler, and/or
• Attend a Chandler Unified School District school
• Be uninsured. Sorry, we cannot accept AHCCCS (Medicaid) patients
• Be birth to 18 years of age. We do serve young adults over the age of 18 if they are enrolled in a CUSD school

Dignity Health Children's Medical Clinic

Medical services provided include:

• Well-checks
• Sports physicals
• Sick visits
• Treatment for minor injury
• Assessment and treatment for chronic conditions
• ADHD screenings

The Clinic is a licensed outpatient facility under Dignity Health and is supported by volunteer providers, grants, donations, and tax credits. For more information on how to support the work done in the CARE Center, please click here.

Dignity Health Children's Dental Clinic

Dental services provided may include:

• Examinations
• X-Rays
• Cleanings
• Treatment for infection
• Oral health education
• Fillings
• Basic extractions
• Root canals and crowns
