Southwest Behavioral & Health Services at the CARE Center
The Southwest Behavioral & Health Services program is a partner in the Chandler CARE Center, providing in-person and virtual appointments for eligible families.
Southwest Behavioral & Health Services (SBH) provides comprehensive behavioral health services on-site at the CARE Center as well as on-site at select CUSD schools. Services are available for youth and their families and include:
- Individualized assessment
- Individualized service plan (ISP)
- Individual and family counseling
- Psychoeducational and therapeutic groups
- and more
If you or someone you know is interested in services through SBHS, the first step is to complete a referral form. Referral forms can be found below.
