    2025-2026 ATHLETIC CLEARANCE - ONLINE JULY 1, 2025
     
    Athletic Clearance Packets for 25-26 fall sports will open on July 1, 2025. All forms must be completed and submitted online through Aktivate (formally Register My Athlete). 
     
    1. You will need to sign up or log in to Aktivate (Be sure to use Google Chrome, Firefox or Safari). Fall sports including Badminton, Cross Country, Flag Football, Football, Golf, Freshman/Transfer Pom and Cheer, Swim/Dive, Freshman Tennis, Unified Sports, Girls Volleyball, and Boys Beach Volleyball will open on July 1, 2025. Winter Sports including Basketball, Soccer and Wrestling will open September 2, 2025.  Spring Sports including Baseball, Softball, Tennis, Track and Field, Girls Beach Volleyball and Boys Volleyball will open January 6, 2026.
    2. AIA Annual Pre-Participation Physical Examination Form (1 page doctor form - must be completed after March 1, 2024).  Note - If you are using Telemed, you still must get a doctor's actual signature or an electronic signature and doctor's office stamp.  Electronic signatures only will not be accepted.
    3. AIA Annual Pre-Participation Physical Evaluation Form (5 page parent and doctor form - must be completed after March 1, 2024). Note - If you are using Telemed, you still must get a doctor's actual signature or an electronic signature and doctor's office stamp.  Electronic signatures only will not be accepted. 
    6. Brainbook Completion Certificate (only needed once during your high school career)
    7. Opioid Education Completion Certificate (only needed once during your high school career)
    8. All athletes must have medical insurance.  If you do not have personal insurance, you can purchase K & K School Accident Insurance.  Please click here for more information.
    9. Unified Sports Participant Release Form (only needed if you are participating in Unified Sports)
    10. For 2024-25 Non-cut sports (Cross Country, Football, Wrestling, Track, and Unified Sports) must download their Activity Fee Payment receipt to Register My Athlete.  This can be paid at the Bookstore or through Infinite Campus.  You can also designate this as part of your Arizona Tax Credit to Public Schools. NOTE: Fees will not be refunded if the student decides not to participate or quits the team.
    12. All other sports - Do not pay your fee until after tryouts are complete and the fee is added to your Infinite Campus In Touch Payments.

    Please consider a tax credit donation to Perry High School Athletics or the sport of your choice. Your tax credit donation directly supports PHS athletes by allowing us to enhance our athletic programs through the purchase of athletic equipment/supplies, uniforms and coaching support.

    Athletic participation fees at PHS range between $60 and $100 per participant. The fees are non-refundable but can be claimed as a Public-School Tax credit on AZ state income taxes. Fees can be paid online via credit card through the Infinite Campus Parent Portal or by cash or check at the PHS Bookstore. Thank you for your support of PHS!

    2024-25 Fall Sports Season Re-Cap

    Badminton - 10-4 Regular Season, 5th Team in state power rankings. Defeated Highland in State Round 1, defeated Xavier in the State Quarterfinals, lost to Hamilton in the State Semi Finals. Alexa Englert/Tina Nguyen made it to the Semi Finals of State Doubles and were named All-Premier Region 1st Team Doubles. Robyn Bui made it to the Quarterfinals of State Singles and was named All-Premier Region 1st Team Singles. Kylie O'Neil and Masie Stoten were named All-Premier Region 2nd Team Doubles. C.C. Hall and Riley Hunt were named All-Premier Region Honorable Mention.  

    Boys Cross Country - 17th Place at State. Caleb Faba was 36th at State and was named All-Premier Region 2nd Team. Roan Milner was 85th.  Allden Buyack was 102nd. Joshua Ozaeta was 116th, Joseph Perez was 118th, Anuj Iyer was 125th, and Maxwell Trickel was 153rd. 

    Girls Cross Country – 6th Place at State. Sectional Runner Up. Ava Withem was 19th at State for All-State 3rd Team and was named All-Premier Region 1st Team. Malia Salcido was 30th and named All-Premier Region 1st Team. Alyssa Hancock was 39th and named All-Premier Region 2nd Team. Danika Torres was 61st and named All-Premier Region 2nd Team. Madilyn Mountain was 66th and named All-Premier Region 2nd Team. Emerson Rhodes was 71stand named All-Premier Region 2nd Team. Maycie Boblitt was 110th.

    Flag Football – 3-9 Regular Season, 29th Team in 6A State Power Rankings. Mia Adams (DB/LB) was named All-6A Conference 2nd Team and All-Premier Region Defensive Player of the Year. Vera Papp (P) was named All-6A Conference 2nd Team and All-Premier Region 1st Team. Isis Willingham (Rusher) and Chloe Beal (DB/LB) were named All-Premier Region 1st Team.  Charlotte Bell (WR), Estella Ewing (Rusher), Sophia Garibay (RB), Taylor Hinton (WR), Mckenna Horne (DB/LB), and Elise Servis (Flex) were named All-Premier Region 2nd Team. Eden Myers and Anailah Perry were named All-Premier Region Honorable Mention.  

    Football – 5-5 Regular Season, 8th Team in 6A State Power Rankings. Won the Honor Bowl. Defeated Chandler in State Round 1. Lost to Brophy in the State Quarterfinals. James Dean (P), Jax Waddell (OL), LJ Walker (WR) and Nate Wheeler (LB) were named All-Premier Region 1st Team. Ethan Austin (DL), Maddox Ford (LB), Jayden Kimling (DB), Simba Mahaso (DB), Hayden Moon (WR), Jackson Sands (LB), Ricky Stewart (DL), and LJ Walker (KR) were named All-Premier Region 2nd Team. Sean Camping, Braidi Glascock, Peyton Grako, Brody Pace, Devin Sarpy, and Kael Snyder were named All-Premier Region Honorable Mention. Seth "Buddy' Graham was named a Phoenix Chapter of the National Alliance of African American Athletes Scholar Athlete of the Year.

    Boys Golf - State Runner Up. Ethan Butters placed 3rd at State and was named All-Premier Region 1st Team. Luke Bratetic placed 18th and was named All-Premier Region 1st Team. Jack McDonough was 23rd and named All-Premier Region 2nd Team. Ty Gould was 29th and named All-Premier Region 2nd Team. Caden Horvath was 34th and named All-Premier Region Honorable Mention. Brady Reynolds was named All-Premier Region Honorable Mention. Scott Uyeshiro was named All-Premier Region Coach of the Year.

    Girls Golf - 4th Place at State. Taylor Underhill placed 3rd at State and was named All-Premier Region 1st Team. Makaleigh Churan was 15th and named All-Premier Region 1st Team. Bella Keselman was 36th and named All-Premier Region 2nd Team. Leah Smiles was 53rd and named All-Premier Region Honorable Mention. Harper Mowry was 61st and named All-Premier Region Honorable Mention.  

    Boys Swim/Dive – Did not score at State. Jack Arnett was 28th in the 500 Free. Alex Gonzales was 32nd in the 50 Free and was named All-Premier Region Honorable Mention. Jacob Rose was 35th in the 200 Free and named All-Premier Region Honorable Mention. The 200 Medley Relay of Ryu Cheng, Gonzales, Rose, and Trent Woods was 23rd. The 400 Freestyle Relay of Arnett, Gonzales, Rose, and Woods was 25th

    Girls Swim/Dive – 23rd Place at State. Mia Rodic was 11th in the 500 Yard Free, 17th in the 200 Yard Free, and was named All-Premier Region Honorable Mention. Samantha Hill was 14th in Dive. Elizabeth Ray was 22nd in the 100 Yard Free, 24th in the 200 Free and named All-Premier Region Honorable Mention. Mckenna Jones was 33rd in the 200 Yard IM. The 200 Free Relay of Alexandra Bacyznski, Jones, Ray, and Rodic was 12th. The 400 Free Relay of Jones, Danica Kunz, Ray, and Rodic was 19th. The 200 Medley Relay of Kunz, Jones, Charlie Mcdonel and Addison Sroufe was 20th.

    Girls Volleyball – 13-4 Regular Season. 9th team in State Power Rankings. Defeated West Point in the State Play-in, lost to Queen Creek in State Round 1. Kenna Cogill (Hitter) was named All-6A Conference 1st Team and All-Premier Region Offensive Player of the Year. Lily LaFlesch (Libero), Kail Shill (Setter), and Avery Stones (Hitter) were named All-Premier Region 1st Team. McKaley Herr (Hitter) was named All-Premier Region 2nd Team. Avery Boothe and Gabby Johnson were named All-Premier Region Honorable Mention.

    Boys Beach Volleyball (Emerging Sport) – 7-1 Regular Season. 3rd team in State Rankings. Had a BYE in State Round 1, defeated Williams Field in the State Quarterfinals, lost to Casteel in State Semi Finals. Grey Carter and Tate Higgins finished 3rd in the Pairs Tournament.

Koa Peat was named a McDonald’s All-American, Arizona Gatorade Player of the Year, Arizona Max Preps Player of the Year, All-6A Conference Player of the Year, All-Premier Region Player of the Year, AZ Central All-Arizona, won a gold medal with the USA U17 Basketball Team and named FIBA All-Tournament 2nd Team, named to the Nike Hoop Summit Men’s USA Team, and named Jersey Mike’s Naismith High School All-America 2nd Team. Nolan Brown was named All-6A Conference Defensive Player of the Year and All-Premier Region Defensive Player of the Year. D'Andre Harrison was named All-6A Conference 1st Team, All-Premier Region 1st Team.  Dominic Avalos and Bruce Branch III were named All-Premier Region 1st Team. Jerrance Howard and Trey McKinney were named All-Premier Region Honorable Mention. Coach Sam Duane was named All-6A Conference and All-Premier Region Coach of the Year.

Lost to Shadow Ridge in the 6A State Finals. Brody Donovan (Forward) was named All-6A Conference 2nd Team and All-Premier Region Offensive Player of the Year. Jacoby Franklin (Defense) was named All-6A Conference 2nd Team and All-Premier Region 1st Team. Jaymis Brown and Nicholas Saveski were named All-Premier Region 1st Team. Ethan Haugan (Defense), Joshau Hurt (Midfield), Andres Perez Hernandez (Goalie), Jack Rode (Forward), and Brayden Weisenfluh (Midfield) were named All-Premier Region 2nd Team. Elijah Bisaha and Brady Silsby were named All-Premier Region Honorable Mention.

Defeated Desert Mountain in the Open State Round 2. Defeated Highland in the Open State Quarterfinals. Lost to Casteel in the Open State Semi Finals. Saydie Herbert (Forward) was named All-6A Conference 1st Team and All-Premier Region Offensive Player of the Year. Ashlyn DeLemos (Midfield) and Nicole Martinez (Defense) were named All-Premier Region 1st Team. Mia Adams (Defense), Ella Griffin (Forward) and Joliye Brass (Midfield) was named All-Premier Region 2nd Team. Layla Deleon and Trinity Lujan were named All-Premier Region Honorable Mention.  

     

    Boys Wrestling – 12th at the State Tournament. Ethan Austin (215lb - Sectional Runner Up) took 4th Place at State and was named All-Premier Region 1st Team. Braidi Glascock (285lb - Sectional Runner Up) took 4th Place at State and was named All-Premier Region Honorable Mention. Caden Briquelet (120lb) took 5th place at State and was named All-Premier Region 1st Team. Adis Wesson (138lb – Sectional Runner Up) qualified for the State Tournament and was named All-Premier Region 1st Team. Landon Dodson (190lb) qualified for the State Tournament and was named All-Premier Region Honorable Mention. Jace Briquelet was named All-Premier Region Honorable Mention. 

     

    Girls Wrestling – 15th at the State Tournament. Addison Palmer (100lb - Sectional Champion) was the State Runner Up and named All-Premier Region 1st Team. Apphia Gutierrez (120lb – Sectional Runner Up) took 4th Place at State and named All-Premier Region 1st Team. Paige Hedeen, Andi Linford, Isabella Montano, and Rose Pope were named All-Premier Region Honorable Mention.

Ryan Davie (Position Player), Andrew Hover (Position Player), and Michael Orbegozo (Pitcher) were named AIl-Premier Region 1st Team. Lou Berg (Position Player), Max DeFrancesco (Position Player) and Rowan Noreus (Position Player) were named All-Premier Region 2nd Team. Dillon Mendoza and Tyler Miller were named All-Premier Region Honorable Mention.  

    Softball – 9-9 regular season. 18th team in state power rankings. Defeated Tolleson in the State Play In Game. Lost to Queen Creek in State Round 1. Lost to Basha in State Elimination Round 1.  Brynlee Brassfield (Position Player), CC Hall (Position Player), Julianna Nava (Pitcher) was named All-Premier Region 1st Team. Hailey Estrella (Position Player), Braylee Harder (Position Player), Ellie Leicht (Catcher),  and Sara Phillip (Pitcher) were named All-Premier Region 2nd Team. Ella Jackman and Sammie Kaufmann-Warner were All-Premier Region Honorable Mention.

    Boys Tennis – 12-2 regular season. 3rd team in state power rankings. Defeated Liberty in State Round 1.  Defeated Rincon/University in the State Quarterfinals. Lost to Brophy in the State Semi Finals. Royce Yeager made it to Round 2 of State Singles and was named All-Premier Region Honorable Mention. Jackson Monroe lost in the 1st Round of State Singles and was named All-Premier Region Honorable Mention. Jensen Holbrook/Dallin Welch made it to the Quarterfinals of State Doubles and were named All-Premier Region 1st Team Doubles. Evan Martin/Gabe Taylor made it to the Quarterfinals of State Doubles and were named All-Premier Region Honorable Mention.  Martin was named All-Premier Region Honorable Mention.

    Girls Tennis – 6-8 regular season. 12th team in state power rankings. Lost to Highland in State Round 1. Bella Taylor lost in the 1st Round of the State Singles and was named All-Premier Region 1st Team. Emma Kaufman-Warner/Taylor Merrill lost in the 1st Round of State Doubles. Mira Jones was named All-Premier Region Singles 1st Team. Maci Clonts, Liliana Hindes, and Kaufman-Warner were named All-Premier Region Honorable Mention.   

    Boys Track - 15th at 6A Division State. Jack Cleary was the Divisional State Champion in the Javelin. Hayden Moon was 4th in the Javelin. Jaymis Brown was 5th in the Triple Jump and 12th in the Long Jump. Roan Milner was 14th in the 1600m.  The 4X100 relay team of Amos Augustine, Vaun Patterson, Max Wendelschafer, and John Yoo placed 15th

    40th at the State Meet. Jaymis Brown was 7th in the Triple Jump. Hayden Moon was 5th in the Javelin. Jack Cleary was 10th in the Javelin.

    Girls Track - 15th at 6A Division State.  Maclane Clark was the state runner up in the 100 Hurdles and 5th in the 300 Hurdles. Ava Withem was 10th in the 3200m and 15th in the 1600m. Linnea Helterbran placed 12th in the Discus. Kathleen Farley placed 14th in the High Jump and 19th in the 300 Hurdles. Danica Torres placed 23rd in the 800m.  The 4x100 Relay team of Clark, Azizah Rahman, Sydney Smith, and Isis Willingham was 11th. The 4x400 Relay team of Clar, Farley, Jilian Jones, and Smith placed 11th. The 4x800 Relay team of Alyssa Hancock, Madilyn Mountain, Danica Torres, and Withem placed 16th.

    31st at the State Meet. Maclane Clark was 6th in 100 Hurdles and 300 Hurdles.

    Beach Volleyball – 5-7 regular season. 17th team in state power rankings. Did not qualify for the state tournament. Ava Witter was named All-Premier Region Offensive Player of the Year. Caitlyn Simmons was named All-Premier Region 1st Team. Alyssa Wurth was named All-Premier Region 1st Team. Hallie McCune and Kesenia Torres were named All-Premier Region Honorable Mention.

    Boys Volleyball – 17-0 undefeated regular season. 1st team in state power rankings. State Champions. Defeated Boulder Creek State Round 1. Defeated Williams Field in the State Quarterfinals. Defeated Higley in the State Semi Finals. Defeated Highland in the State Finals. Logan Gray was named All-6A Conference Player of the Year, All-Premier Region Player of the Year, and AZ Central All-Arizona. Caden Ledbetter (Middle Blocker) and Greyson Carter (Hitter) were named All-6A Conference 1st Team and All-Premier Region 1st Team. Braden Breinholt, Tate Higgins, Zachary Tomlinson, and Everett Wagner were named All-Premier Region 2nd Team.  Kenton Leon and Dax Stones were named All-Premier Region All-Premier Region Honorable Mention.

    ImPACT Testing Has Come To Perry High School
     
    Perry athletes will be taking the ImPACT Baseline Concussion Test for free.  ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing) is the first, most-widely used, and most scientifically validated computerized concussion evaluation system available.  ImPACT was developed to provide useful information to assist qualified practitioners in making sound return to play decisions following concussions.  All athletes in the freshman and junior years will be tested at the beginning of their seasons and if they ever sustain a concussion during the season they will complete a post-test to help determine their readiness to return to play.
     
    Each athlete will need a signed permission slip and waiver in order to participate.  These will come home a few days before the test.  Please be sure to sign and return them promptly.
