Student Intent to Return Form

In order for us to project class sizes next school year (2020-2021) and to plan for the needs of our students, PLEASE complete this online Student Intent to Return Form https://cusdelementaryed.wufoo.com/forms/shumway-student-intent-to-return-2021/   by 8:00 p.m. on Thursday, February 13, 2020.

The Student Intent to Return Form MUST be completed for EVERY student in kindergarten through 5th grade.

2020-2021 Kindergarten Registration begins January 8th. 

• Child must be 5 years by August 31, 2020
• Early entrance testing available - please call the front office for details
• Kindergarten Open House on Tuesday, January 28th, 5:30-6:30pm.  Meet our teachers, tour our school and learn more about Kindergarten.

2020-2021 Preschool Registration begins January 13th.

• Child must be 4 years old by August 31, 2020
• Half day classes:  AM 8:00-11:00,  PM 11:45-2:45
• Space is limited
• Limited availability free of charge for students living within Shumway boundaries. Additionals scholarships are available to students within the Chandler School District.
• Tours available - please call the front office to set up an appointment.

To register in either of these programs, please bring:

• Original Birth Certificate
• Immunization Record
• Proof of Residency, ex: utility bill, lease agreement, etc.
