Welcome to Sanborn Elementary School.


    To officially enroll your child/ren, the following information must be provided:


    ·   Completed immunization record.  All immunizations must be current as per Arizona Health and Safety Code, R9-6-701 – 708.  School immunization requirements can be found on the Arizona Department of Health Services website


    ·       Certified state/government birth certificate.  We are not able to accept hospital issues certificates.


    ·     Proof of residency (i.e. contract of home purchase, lease agreement, utility  bill)


    ·   If applicable, any legal documents, (i.e. custody, guardianship, power of  attorney)


    Please bring all these documents with you at time of registration.  We will make copies for our files and have you complete enrollment paperwork.
