    3 D Mammography Screenings - Chandler School District
     
    To schedule on-line, please use the links below. If you are unable to keep your
    appointment, please call to cancel no later than 1 week prior to the screening event date.

    PATIENTS 35+ CAN BE SEEN WITHOUT A DR'S ORDER.  PATIENTS UNDER 35 MUST PROVIDE A DR'S REFERRAL
     
     MOM unit events have concluded for the 24-25 plan year. 

     25-26 events will be announced via HR Newletter & links posted here once scheduled!

                         Please note:  If the minimum sign-up threshold is not met for a site, there is a possibility of cancelation and a need to re-book.

    If you are experiencing a new lump, new or consistent breast pain or nipple discharge, please consult your provider as you may
    require diagnostic mammogram screening.  You may schedule your diagnostic screening with SimonMed Imaging by calling 1-866-614-8555.

    Non-insured patients pay $235.00
    3D, full-field digital mammography and CAD computerized reading
    Most major insurance carriers accepted: Aetna, Meritain, BCBS, CIGNA, Health Net, UnitedHealthcare, and many more.
    We can accommodate breast implant patients.
