CUSD Records Requests

  • Public records of the District (per Board policy 3-402) will be open for inspection by any person as provided by law. The opportunity to review the records only (no original record may leave the district office), will be made at no cost. Records can only be reviewed during business hours. 

    A fee shall be levied on each request to take copies of the records. Fees will be collected prior to releasing materials and must be paid for in either exact cash (the requestor will be told the amount ahead of time via email), check, or money order.

    The District will not charge a fee for paper copies of:

    • Education records of a student requested by the student or his or her parent
    • Forms and other materials are prepared to assist persons who wish to enroll a child in the District, seek employment by the District, or access services or accommodations from the District 
    • Materials prepared to assist persons who attend District or school meetings
    • Public records needed to assert a claim against the United States, as required by state law

    For all other public record requests for non-commercial purposes, the District will charge a fee of $0.50 per page for paper copies. No electronic copies will be given at this time. Fees must be paid, or acceptable arrangements for payment made, prior to the delivery of public record copies.

    To request records, please print out this form, complete the form and send the form back via email to the Chandler Public Records Department. Send your form in an email to PublicRecordsRequest@cusd80.com. You will receive confirmation shortly that your request has been received. You will be notified of the cost when completed if you desire to get the copies. Otherwise, you will be invited to the CUSD District Office, 1525 W. Frye Road, Chandler, Arizona, 85224 to review the records. Records can only be reviewed during business hours and cannot be taken from the district office unless the fee is paid.     Arizona Revised Statutes (ARS 39-121.03) defines records as: “all books, papers, maps, photographs, or other documentary materials made or received by any governmental agency in pursuance of law or in connection with the transaction of public business and preserved or appropriate for preservation by the agency or its legitimate successor as evidence of the organization, functions, policies, decisions, procedures, operations or other activities of the government, or because of the informational and historical value of data contained in the record.”
