2024/25 AIA Forms

An Urgent Care or Minute Clinic appointment is a

Great way to quickly get a Sports Physical!

Establish or Access an Aktivate Account

Participants must register for each sport individually.

Student Physical Forms must be uploaded to a student's

Aktivate Profile & Approved before they're eligible to tryout!

Overnight/weekend uploads will not be reviewed until the next school day.

Last minute (overnight and weekend) uploads may result in a delayed arrival for tryouts.

In an effort to streamline the Athletic Clearance process, Chandler Unified School District has gone paperless. 

Santan Jr High accepts online registrations for athletes.

 This link contains everything  you need for athletic eligibility for the 2024-2025 seasons.

You may print and take these AIA forms to your physician.

 The AIA Physical forms are also available at the end of the "Aktivate" registration process.

Sports Physicals for the upcoming 2025/2026 school year must be completed after March 1, 2025 and must be on the correct AIA forms.  Physicals done before March 1st will not be eligible for Athletic Clearance.

**All athletics fees will be paid ONLINE though the Infinite Campus Parent Portal!

Santan Junior High is a member of the East Valley Conference, which includes eight other junior highs (including five others from CUSD). Santan sports seasons coincide with our academic quarters.
