    2023-2024 ATHLETIC CLEARANCE - NOW ONLINE
     
    Athletic Clearance Packets are now online for the 2023-24 season.  All forms must be completed and submitted online through Aktivate (formally Register My Athlete). 
     
    1. You will need to sign up or log in to Aktivate (Be sure to use Google Chrome, Firefox or Safari). Fall sports including Badminton, Cross Country, Flag Football, Football, Golf, Freshman/Transfer Pom and Cheer, Swim/Dive, Freshman Tennis, Unified Sports, and Girls Volleyball will open on July 5, 2023. Winter Sports including Basketball, Soccer and Wrestling will open September 11, 2023.  Spring Sports including Baseball, Softball, Tennis, Track and Field, Beach Volleyball and Boys Volleyball will open January 8, 2024.
    2. AIA Annual Pre-Participation Physical Examination Form (1 page doctor form - must be completed after March 1, 2023).  Note - If you are using Telemed, you still must get a doctor's actual signature or an electronic signature and doctor's office stamp.  Electronic signatures only will not be accepted.
    3. AIA Annual Pre-Participation Physical Evaluation Form (5 page parent and doctor form - must be completed after March 1, 2023). Note - If you are using Telemed, you still must get a doctor's actual signature or an electronic signature and doctor's office stamp.  Electronic signatures only will not be accepted. 
    6. Brainbook Completion Certificate (only needed once during your high school career)
    7. Opioid Education Completion Certificate (only needed once during your high school career)
    8. All athletes must have medical insurance.  If you do not have personal insurance, you can purchase K & K School Accident Insurance.  Please click here for more information.
    9. Unified Sports Participant Release Form (only needed if you are participating in Unified Sports)
    10. For 2023-24 Non-cut sports (Cross Country, Football, Wrestling, Track, and Unified Sports) must download their Activity Fee Payment receipt to Register My Athlete.  This can be paid at the Bookstore or through Infinite Campus.  You can also designate this as part of your Arizona Tax Credit to Public Schools. NOTE: Fees will not be refunded if the student decides not to participate or quits the team.
    12. All other sports - Do not pay your fee until after tryouts are complete and the fee is added to your Infinite Campus In Touch Payments.

    Please consider a tax credit donation to Perry High School Athletics or the sport of your choice. Your tax credit donation directly supports PHS athletes by allowing us to enhance our athletic programs through the purchase of athletic equipment/supplies, uniforms and coaching support.

    Athletic participation fees at PHS range between $20 and $60 per participant. The fees are non-refundable but can be claimed as a Public-School Tax credit on AZ state income taxes. Fees can be paid online via credit card through the Infinite Campus Parent Portal or by cash or check at the PHS Bookstore. Thank you for your support of PHS!

    2022-23 Fall Sports Season Re-Cap

    Badminton - 14-0 Regular Season, 1st Team in state power rankings. State Runner Up. Premier Region Champions. Defeated Westwood in State Round 1, Xavier in the State Quarterfinals, Sunnyslope in the State Semi Finals, and lost to Hamilton in the State Finals. Robyn Bui/Amiya Hernandez were the State Doubles Runner Up. Hernandez was named All-Premier Region 1st Team Singles.  Bui was named All-Premier Region 2nd Team. Vienna Morley/Oanh Le made it to the State Doubles Semi Finals and were named All-Premier Region 2nd Team Doubles.Emma James made it to the State Singles Semi Finals, was named All-Premier Region 1st Team Singles, and is a finalist for AIA Scholar-Athlete of the Year. Talisa Chafey was named All-Premier Region 1st Team Singles. Alexa Englert and Tina Nguyen were named All-Premier Region Honorable Mention. 

     

    Boys Cross Country - 17th Place at State. Roan Milner was 47th at State and was named All-Premier Region 2nd Team. Thomas Gauerke was 87th, Grant Bergeson was 90th, Maxwell Waldron was 98th, Joshua Faba was 100th, Timothy Ortiz was 101st, and Tra Chacon was 108th.

     

    Girls Cross Country - 4th Place at State. Isabella Schaller was 13th at State for All-State 2nd Team and was named All-Premier Region 1st Team. Alexis Pagel was 16th for All-State 2nd Team and named All-Premier Region 1st Team. Malia Salcido was 27th and named All-Premier Region 1st Team. Katelyn Munoz was 43rd and named All-Premier Region 2nd Team. Sara Jones was 80th, Ella Adams was 88th, and Cecilia Molovinsky was 99th.

     

    Football - 4-6 Regular Season, 15th Team in 6A State Power Rankings. Lost to Highland in State Round 1. Aiden Herring (D-Line) was named All-Premier Region 1st Team Defense and National Football Foundation Valley of the Sun Chapter Honorable Mention Top Athlete. Landon Sabetta (Long Snapper) was named All-Premier Region 1st Team Specialists. Kyle Bartnik (Wide Receiver) was named All-Premier Region 2nd Team Offense. CJ Snowden (Defensive Back) was named All-Premier Region 2nd Team Defense, National Football Foundation Valley of the Sun Chapter Honorable Mention Top Athlete, and National Alliance for African American Athletes Phoenix Chapter Outstanding Scholar Athlete. Maddox Ford (Linebacker), and Gage Hatch (Linebacker) were named All-Premier Region 2nd Team Defense. Don Tinsely (Kick Returner) was named All-Premier Region 2nd Team Specialists.  Jack Amer, Brock Ford, Andrew Guild, Bo Hall, Marc O'Bannon, and TJ Thompson were named All-Premier Region Honorable Mention. Seth Graham was named National Alliance for African American Athletes Phoenix Chapter Outstanding Scholar Athlete for the Class of 2026. 

     

    Boys Golf - 8th Place at State. Joe Houghton placed 16th at State and was named All-Premier Region 2nd Team. Kyle Koski was 18th and named All-Premier Region 2nd Team. Chris Lavin was 20th and was named All-Premier Region Honorable Mention. Luke Bratetic was 69th. Maxwell Betts was 71st and named All-Premier Region Honorable Mention . 

     

    Girls Golf - 7th Place at State. Gabrielle Cone placed 13th at State and was named All-Premier Region 1st Team. Lili Stutz was 33rd and named All-Premier Region 2nd Team. Jada Franco was 35th and named All-Premier Region 2nd Team. Bella Keselman was 57th and named All-Premier Region Honorable Mention. Ella Hanks competed at State, but did not score and was named All-Premier Region Honorable Mention.

     

    Boys Swim/Dive - Did not place at State. Matthew Johnson was 32nd in the 100 Fly and named All-Premier Region Honorable Mention. The 200 Free Relay of Dayne Christensen, Johnson, Matthew Pearson, and Dylan Sategna was 17th. The 400 Freestyle Relay of Christensen, Johnson, Pearson, and Sategna was 20th. The 200 Medley Relay of Christensen, Johnson, Pearson, and Sategna was 22nd. Dylan Sategna was named All-Premier Region Honorable Mention.  

     

    Girls Swim/Dive - 8th Place at State. Hannah Ernst was State Runner Up in the 100 Backstroke, 13th in the 200 IM, and named All-Premier Region 1st Team in the 100 Back.  Haley Mueller was 4th in the 100 Backstroke, 8th in the 100 Butterfly, and named All-Premier Region 2nd Team in the 100 Back. Samantha Hill was 4th in Dive and named All-Premier Region 1st Team. Sharlotta Mozes was 6th in Dive and named All-Premier Region Honorable Mention. Kiara Terry was 15th in the 500 Free and 21st in the 200 Free. Reese Carpenter was 15th in the 100 Breaststroke and 30th in the 200 IM. Izzy Ricciardi was 16th in the 500 Free, 22nd in the 200 IM and named All-Premier Region Honorable Mention. Sophia Root was 19th in the 200 Free and 30th in the 100 Free. Kiera Lemon was 20th in the 100 Breaststroke and 27th in the 100 Free. Debra Abbott was 24th in the 200 IM. Riley Beall was 32nd in the 100 Backstroke. The 200 Medley Relay of Abbott, Ernst, Lemon and Mueller was 5th and was named All-Premier Region 1st Team. The 400 Free Relay of Abbott, Ernst, Lemon and Mueller was 11th. The 200 Free Relay of Abbott, Beall, Ricciardi and Root was 14th. 

     

    Girls Volleyball -14-3 Regular Season. 4th team in state power rankings. Defeated Desert Vista in State Round 1, O'Connor in the State Quarterfinals, and lost to Corona in the Semi Finals. Avery Burks was named All-6A Conference 1st Team, All-Premier Region Offensive Player of the Year, and was named to the AZ Central All-Arizona Volleyball Team and nominated for Player of the Year. Kenna Cogill (Hitter) was named All-Premier Region 1st Team and AZ Central All-ARizona Volleyball Team Honorable Mention. Reese Biesemyer (Hitter) was named All-Premier Region 1st Team. Amy Wahlin (Hitter), Reagon Reynolds (Setter) and Braelyn Woodworth (Libero) were named All-Premier Region 2nd Team. Kiana Landers and Kate Wagner were named All-Premier Region Honorable Mention.

Koa Peat was named the Arizona Gatorade Player of the Year, Arizona Max Preps Player of the Year, Max Preps Sophomore All-American Team, AZ Preps.com Co-Player of the Year, AIA All-6A Conference Player of the Year, All-Premier Region Player of the Year, was named to the AZ Central All-Arizona Basketball Team, won AZ Central Basketball and Overall Male Player of the Year, and won a gold medal with the USA U16 Basketball Team.  Cody Williams is a McDonald's All-American, was named to the Naismith All-American Team, was named to the 2023 USA Men's Nike Hoop Summit roster, AZ Preps.com Co-Player of the Year, AIA All-6A Conference Offensive Player of the Year,  All-Premier Region Offensive Player of the Year, was named to the AZ Central All-Arizona Basketball Team, and is nominated for Player of the Year. Ben Egbo was named AIA All-6A Conference Defensive Player of the Year, All-Premier Region Defensive Player of the Year, was named to the AZ Central All-Arizona Honorable Mention.  D'Andre Harrison and Barron Silsby were named All-Premier Region 2nd Team. Nolan Brown and Jonas Cederlind were named All-Premier Region Honorable Mention. Coach Sam Duane was named All-6A Conference and All-Premier Region Coach of the Year.

Khamil Pierre was named the Arizona Gatorade Player of the Year, was nominated for McDonald's All-American, has her jersey hung in the Women's Basketball Hall of Fame, AIA All-6A Conference 1st Team, All-Premier Region Player of the Year, was named to the AZ Central All-Arizona Basketball Team, and was nominated for Player of the Year. Lauren Crocker and Halena Hill were named All-Premier Region 1st Team. Jayla Cal and Sage Henry were named All-Premier Region 2nd Team. Isabella Ivy and Hannah Strauss were named All-Premier Region Honorable Mention. Coach Andrew "Mike" Curtis was named All-Premier Region Coach of the Year.

Caden Dosmann was named AIA All-6A Conference Player of the Year, All-Premier Region Player of the Year, was named to the AZ Central All-Arizona Soccer Team, and is nominated for Player of the Year. Tyler Bosi was named AIA All-6A Conference Defensive Player of the Year, All-Premier Region Defensive Player of the Year, and was named AZ Central All-Arizona Soccer Honorable Mention. Hudson Deutsch (Forward) was named AIA All-6A Conference 1st Team, All-Premier Region 1st Team, was named to the AZ Central All-Arizona Soccer Team, and is nominated for Player of the Year. Tyler Breinholt (Midfielder) was named AIA All-6A Conference 1st Team, All-Premier Region 1st Team, and was AZ Central All-Arizona Soccer Honorable Mention. Carson Pridie (Defender) was named AIA All-6A Conference 2nd Team, All-Premier Region 1st Team, and was named AZ Central All-Arizona Soccer Honorable Mention. Tyler Haren (Forwad) and Evangelos Sideris (Forward) were named All-Premier Region 2nd Team, and were named AZ Central All-Arizona Soccer Honorable Mention . Luis Hinogiante (Defender), Aiden Yen (Forward) and Caden Zimmerman (Midfielder) were named All-Premier Region 2nd Team. Aaron Fine and Nicholas Sideris were named All-Premier Region Honorable Mention. Coach Jason Berg was named All-6A Conference Coach of the Year and All-Premier Region Coach of the Year.     

     

    Girls Soccer - 10-2 regular season. 3rd team in state power rankings. State Runner Up. Defeated Gila Ridge in State Round 1. Defeated Pinnacle in State Quarterfinals. Defeated Highland in the State Semi Finals. Lost to Xavier in the State Finals. Siera Herbert (Forward), Joliye Brass (Midfielder), and Kasie Vigil (Defender) were named AIA All-6A Conference 1st Team and All-Premier Region 1st Team. Ella Griffin (Forward), Raegan Hussey (Midfielder), Lexi Saveski (Goalie), Cora Still (Defender), and Sydney Snowden (Defender) were named All-Premier Region 2nd Team. Saydie Herbert and Shayla Hunt were named All-Premier Region Honorable Mention. 

     

    Boys Wrestling – 13th at the State Tournament. Sectional Runner Up. Maximilian Garibay (106lb), Andrew Gutierrez (113lb), Caden Briquelet (120lb), Ferrin Goldstein (132lb), Jayden Kimling (144lb), Jordan Kimling (150lb), Aidan Goodman (175lb), Jacob Gastley (190lb - Sectional Champion), Bo Hall (215lb), and Braidi Glascock (285lb) qualified for the State Tournament. Andrew Gutierrrez took 4th place at State and was named All-Premier Region 1st Team. Maxilmilian Garibay took 5th place at State and was named All-Premier Region 2nd Team. Bo Hall took 6th at State and was named All-Premier Region 1st Team.  Jordan Kimling and Jacob Gastley were named All-Premier Region 1st Team. Caden Briquelet, Ferrin Goldstein, Jayden Kimling, and Aidan Goodman were named All-Premier Region 2nd Team. Connor Cochran and Braidi Glascock were named All-Premier Region Honorable Mention.  

     

    Girls Wrestling – 36th at the State Tournament. Arianna Mauch (152lb - Sectional Runner Up) took 3rd Place at the State Tournament.  Amiya Hernandez (132lb - Sectional Champion) qualified for the State Tournament. 

     

    Spiritline - Show Cheer AIA Division 1 State Champion.  Pom AIA State Runner Up and 4th Place at Nationals. Game Day Cheer 4th Place at AIA State and 10th Place at Nationals. Game Day Pom 6th Place at Nationals.

Lost to Queen Creek in State Elimination Round 2. CJ Snowden (Position Player) was named AIA All-6A Conference 1st Team and AIl-Premier Region 1st Team. Ryan Davie (Position Player), Brayden Tambone (Position Player), and Dominic Jochumsen (Pitcher) and Chris Lavin (Pitcher) were named All-Premier Region 2nd Team. Ethan Miller and Jeremy Sprague were named All-Premier Region Honorable Mention.  Brian Broderick was named All-Premier Region Coach of the Year.
     
    Softball - 14-4 regular season. 4th team in state power rankings. State Runner Up. Defeated to Pinnacle in State Round 1. Defeated Hamilton in State Round 2. Defeated Queen Creek in the State Quarterfinals. Defeated Red Mountain in the State Semi Finals.  Lost to Basha in the State Finals. Rachel Connors (Catcher), Janai Mathis (Position Player), and Alexa Ortiz (Pitcher) were named All-6A Conference 1st Team and All-Premier Region 1st Team.  Noelle Gumm (Position Player), Sienna Kunze (Position Player) and Dominique Walicke (Pitcher) were named All-Premier Region 2nd Team. Talisa Chafey and Sammie Kaufman-Warner were All-Premier Region Honorable Mention.
     
    Boys Tennis - 9-5 regular season. 7th team in state power rankings. Defeated to Mountain View in State Round 1.  Lost to Hamilton in the State Quarterfinals. Dallin Welch made it to the 2nd Round of State Singles and was All-Premier Region Singles 1st Team. Ethan Perovich/Gabe Taylor made it to the 2nd Round of State Doubles. Jensen Holbrook/Weston Webb made it to the 1st Round of State Doubles and were named All-Premier Region Doubles 1st Team.  Luke Plowman, Mitch Rasmussen, and Gabe Tayler were named All-Premier Region Honorable Mention. 
     
    Girls Tennis - 7-7 regular season. 11th team in state power rankings. Defeated Corona in State Round 1. Lost to Desert Vista in the State Quarterfinals. Sydney Park made it to the 2nd Round of State Singles and was All-Premier Region Singles 1st Team.  Brooke Speaker/Callie Whitney made it to the 2nd Round of State Doubles. Kiera Norby/Bella Taylor made it to the 2nd Round of State Doubles. Grace Brenchley, Bella Taylor and Callie Whitney were named All-Premier Region Honorable Mention.   
     
    Boys Track - 14th at 6A Division State. Sam Ford was the State Champion in the 400m and State Runner up in the 200m. Nathan Jacobson placed 7th in the Pole Vault. Hayden Moon was 13th in the Javelin. Pierre Foster was 15th in the High Jump. Ethan Kahler was 15th in the Pole Vault. Chris-Day Kalu-Benjamin was 21st in the Shot Put. Shane Ellard qualified in the Pole Vault but did not clear a height. The 4x800 relay team of Grant Bergeson, Brady Hutchings, Bennet Kaup, and Ran Milner was 9th. The 4x400 relay team of Ford, Evangelos Sideris, Sage Spillsbury, and John Yoo was 11th.  The 4X100 relay team of Nathan Duerkesen, Eli Mesa, Joshua Weatherspon and Elisha Wells was 15th.
    60th at the State Meet. Nathan Jacobson placed 7th in the Pole Vault.  Sam Ford qualified for the 200m and 400m but did not finish. 
     
    Girls Track - 9th at 6A Division State. Larissa Blanchard was the State Runner Up in the 100 Hurdles and 3rd in the Long Jump. Sara Jones was 3rd in the 1600m, 6th in the 3200m, and 10th in the 800m. Alexis Pagel was 8th in the 1600m, 13th in the 3200m, and 18th in the 800m. Vienna Morley was 19th in the Javelin. Savannah Richter was 22nd in the Triple Jump. Emma LeSueur was 24th in the 400m. The 4x800 Relay team of Jones, Londyn Lovell, Pagel, and Isabella Schaller was 3rd. The 4x400 relay team of Maclane Clark, LeSueur, Lovell, and Ashley Sandoval was 13th. The 4x100 relay team of Chloe Brown, Adalynn Buyack, Carrington Little, and Mackenzie Rosenoff was 14th.  
    21st at the State Meet. Larissa Blanchard was 3rd in the 100 Hurdles and 15th in the Long Jump. Alexis Pagel was 10th in the 1600m. Sara Jones was 12th in the 1600m, 13th in the 3200m, and qualified in the 800m but did not finish. The 4x800 Relay team of Jones, Londyn Lovell, Pagel, and Isabella Schaller was 3rd.  
     
    Beach Volleyball – 5-7 regular season. 15th team in state power rankings. Lost to Corona in State Round 1. Avery Burks/Brynn Davie made it to the State Beach Pairs 2nd Round. Avery Burks was named All-Premier Region 1st Team.  Brynn Davie was named All-Premier Region 2nd Team. Ireland Avalos and Kaysie Grittith were named All-Premier Region Honorable Mention.
     
    Boys Volleyball – 17-1 regular season. 2nd team in state power rankings. State Champions. Premier Region Champions. Defeated Red Mountain State Round 1. Defeated Shadow Ridge in the State Quarterfinals. Defeated Brophy in the State Semi Finals. Defeated Highland in the State Finals. Tyler McBurney (Hitter) was named All-6A Conference Player of the Year and All-Premier Region Player of the Year. Ty Baumgarth (Middle Blocker) was named All-6A Conference Defensive Player of the Year and All-Premier Region Defensive Player of the Year. Gunner Cook (Libero) and Mark Tomlinson (Setter) were named All-6A Conference 1st Team and All-Premier Region 1st Team.  Jake Andreason (Hitter) and Caden Ledbetter (Middle Blocker) were named All-Premier Region All-Premier Region 2nd Team.  Caleb Hawkins and Kirk Valadez were named All-Premier Region All-Premier Region Honorable Mention.  Ryan Tolman was named All-6A Conference and All-Premier Region Coach of the Year.

    ImPACT Testing Has Come To Perry High School
     
    Perry athletes will be taking the ImPACT Baseline Concussion Test for free.  ImPACT (Immediate Post-Concussion Assessment and Cognitive Testing) is the first, most-widely used, and most scientifically validated computerized concussion evaluation system available.  ImPACT was developed to provide useful information to assist qualified practitioners in making sound return to play decisions following concussions.  All athletes in the freshman and junior years will be tested at the beginning of their seasons and if they ever sustain a concussion during the season they will complete a post-test to help determine their readiness to return to play.
     
    Each athlete will need a signed permission slip and waiver in order to participate.  These will come home a few days before the test.  Please be sure to sign and return them promptly.
