HELMET, SHOULDER PADS, ETC. DONATION FORMS
All parent purchased helmets, shoulder pads, etc. need to be donated to the district prior to use in practice or games. Please click on the forms below, download and fill them out, and return the forms, your receipt, and the item being donated to the HHS equipment manager, Donnie Sylvestre. Once approved by Mr. Sylvestre, the donation request will be sent to the CUSD Governing Board for final approval. No items may be used until this process is complete.
